The Details of Mother/Person Who Gave Birth to the Child page must be completed by the mother/person who gave birth to the child. If this is not possible, do not complete this registration online;  Contact Us for instructions.

If a father/co-parent is being recorded on this birth registration, that parent and the mother/person who gave birth to the child must both be present to complete and certify the online registration.  A father/co-parent must be the biological parent of the child or the spouse or adult interdependent partner of the mother/person who gave birth to the child.

If the father/co-parent is not present, their details cannot be recorded on this birth registration and will not appear on a birth certificate. A father/co-parent can be added later to a birth record with an amendment; Contact Us for details.

Recording a father/co-parent without them certifying their information and the child’s information is an offence.

Have you already completed a paper Registration of Birth form for your child, and left it with the hospital or your registered midwife?
Have you already submitted your baby's birth information through the Online Birth Registry?
Child's date of birth. If your child's date of birth is more than 1 year ago, please Contact Us.
Was the mother/person who gave birth to the child a surrogate?
Are you registering the birth of a child you are adopting?

To register a birth online, there is a limit to the number of characters that can be entered (character count includes letters, hyphens, periods, apostrophes and blank spaces between names).

Example: Helen Jo-Anne = 13 characters

Example: L’Heureux-St. Moritz = 20 characters

  • Parents:
    • 68 character spaces are available for the last name
    • 68 character spaces are available for a given name (this is the first name and all the middle names in total)
  • Child:
    • 68 character spaces are available for the last name
    • 40 character spaces are available for a given name (this is the first name and all the middle names in total)
Will you need to record a name with more characters than are allowed as noted above?
Is this a single or multiple birth?

A person may have

  • a given name (first name and all middle names) and last name, or
  • only one name (a mononym) when culturally appropriate.

When a person’s culture does not distinguish between a given name (first and all middle names) and a last name, that one name is called a mononym. A person's mononym may have a single word (e.g., Madonna) or more than one word (e.g., Soaring Grey Eagle).

Do you, the other parent or your child have a mononym?

If a father/co-parent is being recorded on this birth registration, that parent and the mother/person who gave birth to the child must both be present to complete and certify the online registration. A father/co-parent is the biological parent of the child or the spouse or adult interdependent partner of the mother/person who gave birth to the child.
